Influence of Role Conflict, Nursing Professionalism, and Job Satisfaction on Turnover Intention of Clinical Nurses with Physician Assistant Role

Abstract
Purpose: The purpose of this study is to investigate the factors influencing role conflict, nursing professionalism, job satisfaction, and turnover intention of Clinical Nurses with Expanded Roles (CNER) in Korea.

Methods: Data were collected from 170 CNER who worked at four general hospitals in Korea. Structured questionnaires were used to assess role conflict, nursing professionalism, job satisfaction, and turnover intention of CNER. The data were analyzed using descriptive statistics, t-tests, ANOVA, Scheffe post hoc analysis, Pearson¡¯s correlation coefficient, and hierarchical multiple regression analysis.

Results: Role conflict (r=.38, p<.001) had a significantly positive relationship with turnover intention. Age (¥â=-.21, p=.004), shift work (¥â=.24, p=.001), and role conflict (¥â=.40, p<.001) were factors influencing turnover intention. This regression model explained 24% of the variance (F=10.15, p<.001).

Conclusion: Important issues include how the role of the CNER is highly conflicted and how intention to change jobs is high, and these issues should be discussed in terms of nurse staff management. Since no related legal system exists in Korea, efforts should be made to solve these problems with CNER by establishing a system of nurse practitioners.
